Fireworks blasts kill at least 24 near Mexico City

MEXICO CITY (Reuters) – Two explosions at fireworks workshops outside Mexico City on Thursday killed at least 24 people, including rescue workers, and injured dozens more, officials said, in the latest deadly blast to hit a town known for its fireworks production.

After a first blast in the municipality of Tultepec, firefighters, police and other rescue workers arrived at the scene when a second explosion occurred, the state government said in a statement.

“Emergency crews attended the call of the first explosion, when a second incident occurred, killing and injuring members of these groups,” the statement said.

Television images showed a plume of smoke rising over buildings on the outskirts of Tultepec and scores of firefighters and rescue workers at the scene.

The attorney general’s office for the state of Mexico, the country’s most populous state which rings the capital, said that 17 people had died at the blast site and another seven died in hospital.

Another 49 people were injured, the statement added.

A series of blasts have taken occurred at the fireworks markets, workshops and depots in Tultepec, about 20 miles (32 km) north of Mexico City, including massive explosions in a market in December 2016 that killed around three dozen people.

Luis Felipe Puente, the head of Mexico’s civil protection agency, said the sale of fireworks in the area would be suspended and permits of manufacturers would be reviewed.

HONOLULU, Hawaii (Reuters) – A small explosion at the summit of Hawaii’s erupting Kilauea Volcano on Sunday sent ash spewing into the air, creating a driving hazard for roads on parts of the Big Island, the U.S. Geological Survey said.

Lava fountains from a fissure in the volcano reached as high as 180 feet (55 meters) from Saturday night into Sunday, pushing flows of molten rock into the ocean, it said.

“Seismic activity at the crater continues with gas explosions and ash eruptions under 10,000 feet (3,050 meters). While the eruption is never predictable, conditions appear stable for the moment,” Richard Rapoza, a spokesman for Hawaii Emergency Management Agency, said in an email.

The eruption, which entered its 39th day on Sunday, stands as the most destructive in the United States since at least the violent 1980 eruption of Mount St. Helens in Washington state that reduced hundreds of square miles (km) to wasteland and killed nearly 60 people, according to geologist Scott Rowland, a volcanologist at the University of Hawaii at Manoa.

No one has died in this Hawaii eruption but some 600 homes have been swallowed by lava flows from Kilauea since May 3, Hawaii County Mayor Harry Kim said last week.

Vacationland, a private development believed to comprise about 160 homes, was completely erased, and at least 330 houses were devoured by lava at Kapoho Beach Lots, Kim said.

SAN MIGUEL LOS LOTES, Guatemala (Reuters) – Guatemalan officials warned of falling ash from the Fuego volcano late on Thursday and urged caution with flights as the Central American country recovers from devastating eruptions that have killed at least 109 people.

The seismological, volcanic and meteorological institute Insivumeh advised the civil aviation authority to take precautions with flights amid renewed activity from the peak, which produced a massive eruption on Sunday.

The death toll from Fuego’s most violent eruption in four decades has been gradually rising and now stands at 109, the Guatemala’s disaster and forensic agency Inacif said earlier on Thursday.

Authorities have said a communication breakdown between CONRED and volcanologists in Guatemala delayed evacuations from the surrounding area.

Guatemala’s public prosecutor said on Thursday it would open an investigation into whether protocols were followed to inform proper decision-making in the handling of the disaster.

Rescue teams have been searching frantically for survivors and victims in the ravaged landscape, which is covered in ash and lava.

The eruptions have showered volcanic ash over a vast area and spewed deadly, fast-moving pyroclastic flows through nearby towns.

The U.S. government expressed its “deepest condolences” to the victims on Thursday and said it was sending emergency aid at Guatemala’s request, including an unspecified amount of financial resources to help with food, water, and sanitation.

The White House said in a statement it was also dispatching aircraft to transport burn victims for treatment in Florida.

The International Federation of Red Cross and Red Crescent Societies (IFRC) raised concerns about the economic cost of the disaster in the poor country.

“We should not underestimate the scale of this disaster. Critical, emergency needs are still enormous, and affected communities will need sustained and long-term support,” IFRC President Francesco Rocca said in a statement on Thursday.

Rocca noted that ash had fallen across more than half of Guatemala, covering areas where agriculture is crucial.

“We hope it will not mean a secondary disaster,” he said.

The IFRC has pledged more than 250,000 Swiss francs ($253,000) to support rescue efforts and said those worst hit would need at least a year to recover.

The suspension of rescue efforts around the volcano may be lifted if conditions on the ground improve, CONRED said.

Volcan de Fuego, which means “Volcano of Fire” in Spanish, lies about 25 miles (40 km) southwest of the capital, Guatemala City.

HONOLULU (Reuters) – The Hawaii community hardest hit by the Kilauea Volcano was ordered sealed off under a strict new mandatory evacuation on Thursday as the eruption marked its fourth week with no end in sight.

The Big Island’s mayor, Harry Kim, declared a roughly 17-block swath of the lava-stricken Leilani Estates subdivision off-limits indefinitely and gave any residents remaining there 24 hours to leave or face possible arrest.

The mandatory evacuation zone lies within a slightly larger area that was already under a voluntary evacuation and curfew.

The latest order was announced a day after police arrested a 62-year-old Leilani Estates resident who fired a handgun over the head of a younger man from the same community, apparently believing his neighbor was an intruder or looter.

The confrontation on Tuesday was recorded on cell phone video that later went viral.

But the mandatory evacuation was “decided prior to that incident,” said David Mace, a spokesman for the Federal Emergency Management Agency currently assigned to the Hawaii County Civil Defense authority.

Civil defense officials have previously said about 2,000 residents in and around Leilani Estates were displaced at the outset of the current eruption, which began on May 3.

But the total number of evacuees was estimated to have risen to about 2,500 after authorities ushered residents from the nearby Kapoho area as a precaution on Wednesday, as a lava flow threatened to cut off a key access road.

At least 75 homes — most of them in Leilani Estates — have been devoured by streams of red-hot molten rock creeping from about two dozen large volcanic vents, or fissures, that have opened in the ground since Kilauea rumbled back to life four weeks ago. Lava flows also have knocked out power and telephone lines in the region, disrupting communications.

Besides spouting fountains of lava around the clock, the fissures have released high levels of toxic sulfur dioxide gas on a near constant basis, posing an ongoing health hazard. Meanwhile, the main summit crater has periodically erupted in clouds of volcanic ash that create breathing difficulties and other problems for residents living downwind.

The heightened volcanic activity has been accompanied by frequent earthquakes, as magma — the term for lava before it reaches the surface — pushes its way up from deep inside the earth and exerts tremendous force underground.

After a month of continual eruptions at Kilauea’s summit and along its eastern flank, geologists say they have no idea how much longer it will last.

“There’s no sign we’re getting that anything is going to slow down at the moment,” Wendy STOVL, a vulcanologist for the U.S. Geological Survey, told reporters on a conference call on Thursday. “We don’t see any changes occurring.”

The island’s mayor on Wednesday renewed an emergency proclamation for 60 more days, allowing construction of temporary shelters and other relief projects to proceed on an expedited basis, without reviews and permits normally required.

The month-old eruption of Kilauea, one of the world’s most active volcanoes, followed an eruption cycle that had continued almost nonstop for 35 years.

Stovall said geologists now believe the latest upheaval should be classified as a separate volcanic event, though an official determination has yet to be made.

HAVANA (Reuters) – More than 100 people were killed in a fiery crash of a Boeing 737 passenger plane in Cuba on Friday, with just three seriously injured survivors in hospital after being pulled from the wreckage, officials and state media said.

The aircraft, on a domestic flight to Holguin in eastern Cuba, crashed shortly after taking off from Havana at 11 a.m. (1500 GMT). There were either 104 or 105 passengers, including five children, plus nine crew members, various state media said.

“We should expect that the news will not be good, as there are a high number of people who appear to have been killed,” President Miguel Diaz-Canel said in broadcast comments.

The fire from the crash had been put out and authorities were identifying bodies, the president said. The cause of the crash was not immediately known and Diaz-Canel said authorities were investigating.

The Boeing 737-201 aircraft was built in 1979 and leased by Cuban airline Cubana from a small Mexican firm called Damojh, according to the Mexican government. That would make it significantly older than most planes in service.

Damojh in Mexico said it did not immediately have any more information. Cubana declined to comment.

Cubana has been the subject of complaints over service and delays in recent months, according to state media.

Wreckage of Flight CU972 was strewn over the crash site area 20 km (12 miles) south of Havana, a Reuters witness said, and blackened parts of the fuselage were visible. Its destination Holguin is the capital of a province that is popular with tourists for its pristine beaches.

“We heard an explosion and then saw a big cloud of smoke go up,” said Gilberto Menendez, who runs a restaurant near the crash site in the agricultural area of Boyeros.

Carlos Alberto Martinez, the director of Havana’s Calixto Garcia hospital, told Reuters that four victims of the accident had been brought there. One had died and three others, all women, were in a serious condition, he said.

“She is alive but very burnt and swollen,” said a distressed relative of one of the survivors at the hospital.

The reason for the plane going down was unclear. “During take-off (the plane) apparently suffered a problem and dived to the ground,” the Mexican transport department said on its website.

Most aircraft accidents take months of investigation to explain and are typically caused by a cocktail of different factors, according to aviation experts.

CUBANA COMPLAINTS

Boeing Co said in a Twitter post: “We are aware of news reports out of Cuba and are closely monitoring the situation.”

Boeing 737 aircraft use engines made by CFM International, the supplier of the world’s most-used engines, built by a joint venture of GE and France’s Safran.

On Thursday, Cuba’s First Vice President Salvador Valdés Mesa had met with Cubana bosses to discuss public complaints about its service, according to state-run media. Problems included the cancellation of numerous domestic flights this year, and long delays which the company said were caused by technical problems with its aircraft.

Earlier this month, the company was ordered to suspend flights by its six Russian built AN-158 aircraft, of which most had reportedly already been grounded.

The last fatal crash in Cuba was in 2017, the Aviation Safety Network said. It was a military flight that killed all eight on board. In 2010, a commercial Aero Caribbean plane crashed in central Cuba. All 68 people on board were killed.

The latest available information on Cuba from U.N. safety aviation agency ICAO, dating back to 2008, ranks it above the global average, though that preceded the latest three crashes.

‘Ash fallout’ alert after Hawaii volcano erupts in 30,000-foot plume

PAHOA, Hawaii (Reuters) – An explosive eruption spewed ash 30,000 feet (9,144 meters) into the air above Hawaii’s Kilauea volcano on Thursday and residents of the Big Island were warned to take shelter as the plume engulfed a wide area, authorities said.

The wind could carry the ash plume as far as Hilo, the Big Island’s largest city and major tourism center, the County of Hawaii Civil Defense warned in an alert.

“Protect yourself from ash fallout,” it said.

The 4:15 a.m. (10:15 a.m. ET) blast that sent ash and smoke nearly six miles into the atmosphere was followed by other emissions of up to 12,000 feet, the U.S. Geological Survey said in a statement.

USGS geologists and staff were evacuated from the summit shortly before the blast and a webcam showed a gray plume of ash and chunks of magma known as pyroclasts that showered the volcano’s slopes.

An aviation red alert was issued due to risks that ash could be carried into aircraft routes and damage jet engines, USGS said.

The eruption could not only enshroud large areas of the Big Island in volcanic ash and smog but other Hawaiian Islands and potentially distant areas if the plume reaches up into the stratosphere and ash is carried by winds.

National Guard troops donned gas masks to protect themselves from toxic sulfur dioxide gas at the intersection of highways 130 and 132, the main exit routes from the village of Pahoa, 25 miles (40 km) east of the volcano, where many of the ground fissures have erupted, a Reuters reporter in the village said.

Schools were closed in the area due to “elevated sulfur dioxide (SO2) levels,” according to a phone alert from emergency authorities

The volcano has destroyed at least 37 homes and other structures in a small southeast area of the island where lava has oozed from fissures, forcing around 2,000 people to evacuate their homes.

Geologists had warned explosive eruptions could begin once Kilauea’s falling lava lake descended below the water table, allowing water to run on to the top of the lava column and create steam-driven blasts.

The powerful explosions could hurl “ballistic blocks” the size of refrigerators across a distance of more than half a mile (1 km) and shoot pebble-sized projectiles and debris up to a dozen miles, the USGS has warned.

Kilauea, one of the most active volcanoes in the world, last experienced explosive eruptions in 1924.

Militant family uses child in suicide bomb attack on Indonesian police

SURABAYA, Indonesia (Reuters) – A family of Islamist militants in Indonesia carried an eight-year-old into a suicide bomb attack against police in Surabaya on Monday, a day after another militant family killed 13 people in suicide attacks on three churches in the same city.

The suicide bombers rode two motorbikes up to a checkpoint outside a police station and blew themselves up, police chief Tito Karnavian told a news conference in Indonesia’s second-largest city.

He said the child survived the explosion, and CCTV footage showed the girl stumbling around in the aftermath.

Four officers and six civilians were wounded in the attack, East Java police spokesman Frans Barung Mangera said.

“We hope the child will recover. We believe she was thrown 3 meters (10 ft) or so up into the air by the impact of the explosion and then fell to the ground,” said Mangera, adding she had been rushed to hospital.

President Joko Widodo branded the attacks in Surabaya the “act of cowards”, and pledged to push through a new anti-terrorism bill to combat Islamist militant networks.After some major successes tackling Islamist militancy since 2001, there has been a resurgence in recent years, including in January 2016 when four suicide bombers and gunmen attacked a shopping area in the capital, Jakarta.

Police suspected Sunday’s attacks on the churches were carried out by a cell of the Islamic State-inspired group Jemaah Ansharut Daulah (JAD), an umbrella organization on a U.S. State Department terrorist list that is reckoned to have drawn hundreds Indonesian sympathizers of Islamic State.

“In the case of Surabaya, they escaped detection, but once it happened we moved fast to identify their network,” Karnavian said.

The father of the family involved in those attacks was the head of a JAD cell in the city, the police chief said.

Earlier, police said his family was among 500 Islamic State sympathizers who had returned from Syria, but the police chief said that was incorrect.

During the hunt for the cell, police shot dead four suspects and arrested nine, media reported police as saying.

The police chief said the JAD cell may have been answering a call from Islamic State in Syria to “cells throughout the world to mobilize.”

He said the imprisonment of JAD’s leader, Aman Abdurrahman, could be another motive, and cited clashes with Islamist prisoners at a high-security jail near Jakarta last week in which five counter-terrorism officers were killed.

Karnavian said the JAD attacks used a powerful home-made explosive triacetone triperoxide (TATP), known as the “mother of Satan”, and commonly used in Islamic State-inspired attacks.

In another incident in Sidoarjo, south of Surabaya, police recovered pipe bombs at an apartment where an explosion killed three members of a family alleged to have been making bombs, Karnavian said. Three children from the family survived and were taken to hospital.

In all, 31 people have died since Sunday in attacks, including 13 suspected perpetrators and 14 civilians, police said.

CHILDREN USED IN ATTACKS

CCTV footage of the blast outside the police station early on Monday morning showed two motorbikes arriving at a checkpoint next to a car followed by an explosion as officers approached.

Security experts said the attacks represented the first time in Indonesia that children had been used by militants on a suicide mission.

“The objective of using a family for terror acts is so it is not easily detected by the police,” said Indonesian security analyst Stanislaus Riyanta.

He said that families could also avoid communicating using technology that could be tracked.

Indonesia’s chief security minister said that police backed by the military would step up checks across Indonesia.

In Surabaya, police officers wearing balaclavas were posted at major hotels and landmarks on Monday.

President Widodo said he would issue a regulation in lieu of a new anti-terror law next month if parliament failed to pass the bill.

Police have complained that laws do not give them enough powers to detain suspects to prevent attacks.

Speaker of parliament Bambang Soesatyo told Metro TV that the house was committed to wrap up debate on the bill this month, but called on the government to help resolve differences.

Wisconsin city lifts evacuation order after refinery explosion

MILWAUKEE, Wis. (Reuters) – Tens of thousands of residents of a northern Wisconsin city were cleared to return to their homes on Friday, the day after a blast at a Husky Energy Inc refinery injured at least 15 people, a local official said.

Investigators searched for the cause of the massive Thursday morning explosion at the refinery, capable of processing up to 38,000 barrels of oil a day, which shook the city of Superior, Wisconsin, home to about 27,000 people.

“All indications are that the refinery site is safe and stable and the air quality is clean and normal,” Superior Mayor Jim Paine said in a Facebook posting, noting that the evacuation order was lifted as of 6 a.m. local time (1100 GMT). “Welcome home.”

At least 15 people were injured, local media reported, and at least 10 people – one seriously injured – were taken to area hospitals, said a spokeswoman for Essentia Health-St. Mary’s Medical Center, which operates hospitals in Superior and nearby Duluth, Minnesota.

What ignited the blast was not clear. After an initial blaze was extinguished, a storage tank was punctured, and a second fire erupted, Husky Energy spokesman Mel Duvall said.

Another tank caught fire at 3:15 p.m., a local ABC affiliate reported, citing Douglas County authorities.

“Our focus in the days ahead will turn to the investigation and understanding the root cause of the incident,” Husky Energy said in a late Thursday Twitter post.

Thick black smoke billowed from the facility and hung over Superior throughout the day on Thursday, forcing tens of thousands to flee homes and businesses.

Friday classes were canceled in Superior and nearby Maple school districts.

There were no reports of fatalities, and all of the refinery’s workers have been accounted for, Husky Energy’s Duvall said.

The refinery had additional workers on site preparing for a plant-wide overhaul when the blast occurred, he said. It produces asphalt, gasoline, diesel and heavy fuel oils, largely using heavy crude oil imported from Canada.

The U.S. Chemical Safety and Hazard Investigation Board has sent a four-person team to investigate. The non-regulatory federal agency investigates serious chemical accidents such as refinery fires.

Husky purchased the refinery from Calumet Specialty Products Partners LP last year.

(Reuters) – The U.S. Federal Aviation Administration said it will order inspection of about 220 aircraft engines as investigators have found that a broken fan blade touched off an engine explosion this week on a Southwest flight, killing a passenger.

The regulator said late on Wednesday it plans to finalize the air-worthiness directive within the next two weeks. The order, which it initially proposed in August following an incident in 2016, will require ultrasonic inspection within the next six months of the fan blades on all CFM56-7B engines that have accrued a certain number of takeoffs.

Airlines said that because fan blades may have been repaired and moved to other engines, the order would affect far more than 220 of the CFM56-7Bs, which are made by a partnership of France’s Safran <SAF.PA> and General Electric <GE.N>.

The CFM56 engine on Southwest <LUV.N> flight 1380 blew apart over Pennsylvania on Tuesday, about 20 minutes after the Dallas-bound flight left New York’s LaGuardia Airport with 149 people on board. The explosion sent shrapnel ripping into the fuselage of the Boeing 737-700 plane and shattered a window.

Bank executive Jennifer Riordan, 43, was killed when she was partially pulled through a gaping hole next to her seat as the cabin suffered rapid decompression. Fellow passengers were able to pull her back inside but she died of her injuries.

On Wednesday, National Transportation Safety Board Chairman Robert Sumwalt said the incident began when one of the engine’s 24 fan blades snapped off from its hub. Investigators found that the blade had suffered metal fatigue at the point of the break.

Sumwalt said he could not yet say if the incident, the first deadly airline accident in the United States since 2009, pointed to a fleet-wide problem in the Boeing 737-700.

Southwest crews were inspecting similar engines the airline had in service, focusing on the 400 to 600 oldest of the CFM56 engines, according to a person with knowledge of the situation. It was the second time this kind of engine had failed on a Southwest jet in the past two years, prompting airlines around the world to step up inspections.

A NTSB inspection crew was also combing over the Boeing <BA.N> 737-700 for signs of what caused the engine to explode.

Sumwalt said the fan blade, after suffering metal fatigue where it attached to the engine hub, has a second fracture about halfway along its length. Pieces of the plane were found in rural Pennsylvania by investigators who tracked them on radar. The metal fatigue would not have been observable by looking at the engine from the outside, Sumwalt said.

Passengers described scenes of panic as a piece of shrapnel from the engine shattered a plane window, almost sucking Riordan out.

Riordan was a Wells Fargo <WFC.N> banking executive and well-known community volunteer from Albuquerque, New Mexico, the company said.

Videos posted on social media showed passengers grabbing for oxygen masks and screaming as the plane, piloted by Tammie Jo Shults, a former U.S. Navy fighter pilot, prepared for the descent into Philadelphia.

The airline expected to wrap up its inspection of the engines it was targeting in about 30 days.

The GE-Safran partnership that built the engine said it was sending about 40 technicians to help with Southwest’s inspections.

Pieces of the engine including its cowling – which covers its inner workings – were found about 60 miles (100 km) from Philadelphia airport, Sumwalt said. The investigation could take 12 to 15 months to complete.

In August 2016, a Southwest flight made a safe emergency landing in Pensacola, Florida, after a fan blade separated from the same type of engine and debris ripped a hole above the left wing. That incident prompted the FAA to propose last year that similar fan blades undergo ultrasonic inspections and be replaced if they failed.

‘Angels’ and training help former fighter pilot save Southwest flight

(Reuters) – The pilot who safely landed a stricken Southwest Airlines flight on Tuesday got her first flying experience in the U.S. Navy, touching down F-18 fighter jets at 150 miles per hour on aircraft carriers.

Tammie Jo Shults, 56, may have drawn on her Navy skills when one of the two engines on her Boeing 737-700 blew and broke apart at 32,000 feet on Tuesday, forcing her to implement a rapid descent toward Philadelphia International Airport.

The explosion killed one passenger and nearly sucked another out of a shattered window.

One of the first female fighter pilots in the U.S. Navy, Shults calmly told air traffic control that part of her plane was missing, and she would need ambulances on the runway.

“So we have a part of the aircraft missing so we’re going to need to slow down a bit,” Shults told a controller.

Many of the 144 passengers sang her praise on social media after Shults thanked them for their bravery as they left the plane.

“The pilot Tammy Jo was so amazing! She landed us safely in Philly,” said Amanda Bourman on Instagram.

Passengers identified Shults as the pilot. Southwest Airlines declined to name the crew of flight 1380 and Shults was not immediately available for comment.

Authorities said the crew did what they were trained to do.

“They’re in the simulator and practice emergency descents..and losing an engine… They did the job that professional airline pilots are trained to do,” National Transportation Safety Board Chairman Robert Sumwalt told reporters.

“GOD SENT HIS ANGELS”

Shults might never have become a pilot if she had not been so determined to fly from a young age.

She is quoted on fighter plane blog F-16.net saying she tried to attend an aviation career day at high school but was told they did not accept girls.

A native of New Mexico, she never lost the urge to fly and, after studying medicine in Kansas, applied to the Air Force. It would not let her take the test to become a pilot, but the U.S. Navy did.

She was one of the first female F-18 pilots and became an instructor before she left the Navy in 1993 and joined Southwest, according to the blog.

A Christian, who is married to a fellow pilot and has two children, Shults said that sitting in the captain’s chair gave her “the opportunity to witness for Christ on almost every flight.”

Bourman was among passengers who said they had been saved by divine intervention.

“God sent his angels to watch over us,” she said.
